Two-Year Limited Product Warranty

Care4MyHealth Fluid-Motion Comfort Insoles include a two-year limited warranty covering defects in materials or workmanship under normal use.

To register your warranty, complete the Warranty Registration form within 30 days of purchase. Keep the original dated receipt or other acceptable proof of purchase.

This limited warranty does not cover:

• •Normal wear and tear
• •Accidental punctures or cuts
• •Damage caused by misuse or improper care
• •Damage caused by cutting or altering the product
• •Damage caused by incompatible or improperly fitted footwear
• •Improper washing, drying or storage
• •Loss or theft
• •Individual fit or comfort preferences
• •Any claimed health-related outcome

If a product is confirmed to have a covered defect, Care4MyHealth may provide a replacement in accordance with the terms of this limited warranty.

After the first six months from the original purchase date, applicable shipping and handling charges may be required. Contact Care4MyHealth before returning a product to receive current instructions.

Return products only after receiving instructions from customer service. Include your name, contact information, proof of purchase and a description of the product issue.
